Wellness Journal – Physical and Mental Health Tracker

I made this out of desperation, honestly. I’ve always had food aversions, allergies, and sensory issues. As I’ve gotten older, things have gotten progressively worse—with no real answers from any of the “professionals” I’ve gone to over the decades. Most providers were content making me feel fat, lazy, and crazy, or only offering overly simplified suggestions that never helped. If they mentioned keeping a wellness journal, it was equally vague: “Just track things”—with zero guidance on what to actually track.
Being neurodivergent, I often struggle to find words for what’s happening in my body in the moment. So “just write it down”, left me staring blankly at a page for hours.
So I started building a system with fill-ins and circle options so I’d never have to face a blank page again. What wrecked my body wasn’t always unhealthy food. It could be a neighbor’s scented candles, cigarette or marijuana smoke coming through apartment walls, fermented foods everyone swore would heal my gut but actually destroyed it, where I was in my menstrual cycle, whether I’d actually slept or just dissociated for eight hours, etc. It’s all connected, and nobody handed me that roadmap. So I made one for myself. For my survival.
I knew it was either figure this shit out or die in ten years. But listening to providers shrug like my constant pain and ill health wasn’t a big deal, or that it would be “simply” resolved if I took this med or that med that only made things worse? That’s what changed everything.
And it helped tremendously. When an Immunologist swore I didn’t have MCAS, my wellness log told a different story. When providers in my past pushed SSRIs for anxiety and depression—claiming I was just “making it up” when I suffered every so-called ‘rare’ side effect—this tracker revealed the truth: food and vitamin deficiencies were often the biggest drivers of my mental health struggles.
Armed with that data, I started pushing my PCPs to check for specific markers commonly overlooked in blood work. This has been the best tool I’ve found for finally seeing the full picture and taking better care of my physical and mental health.
It won’t diagnose you, and shouldn’t replace a medical provider. But it might give you the evidence you need when someone isn’t fully listening or doubts you know more about your body than they do—which is sometimes the only power you’ve got when the system keeps failing you because your body works differently than most people understand.
Two versions available — one with Chakra tracking and one without. Pick and choose what sections are helpful.
